Lesbian and Single Women have been discriminated against and
denied the opportunity to use clinics for artificial insemination,
they have resorted to locating a donor or even advertising.
The company and this unique service evolved out of this to
assist as a conduit for both parties and ensuring the donor
is genuine, fully screened and tested and as safe as possible.
Our non-disclosure policy to ensure anonymity is secure is
paramount for the existence of the company and both Donors
and Recipients

| Q: Which details of your donors will be revealed? |
| A: Specifically we provide
details of age, height, weight, hair and eye colour, sexual
preference, status, ethnicity, country of origin, current
city of residence. We ask for blood group and their religion
however, they often exclude the answer because not all of
them know their blood grouping and some were born with a religion
that they no longer practise. If either of these points is
important to you we can carry out a blood group test for you
on the donor (Subject to an additional charge). We can ask
him what his birth religion was. |

| Q: How do I / we register as recipient(s)? |
| A: You must firstly apply
to be registered with Fertility4Life. To register your
application, click on the Recipient Registration button from the
main menu. You will receive an automatic email immediately
confirming our receipt of your application. If you do not
receive confirmation, register again ensuring that you have
entered your correct email address. If you still have problems,
email us at info@Fertility4Life.com.com
and include a contact telephone number. Our staff will review
your application, and will advise you whether or not it has
been accepted (usually within 72 hours). |

| Q: I try to register and am told I cannot register twice. |
| A: You are already registered
- although your application may not be fully complete. Login
to profile maintenance using your unique ID, and your email
address to maintain (complete) your application. |

| Q: What tests are performed on donor sperm? |
| A: We will carry out a full fertility test on all donors and the results will be supplied to you in writing, our testing is carried out by accredited testing centres throughout the UK .
The S.T.D.s tested for are:-
Blood
VDRL-Syphilis Trichomonas , Hepatitis B & C and
HIV 1 & 2 antibodies (& HIV p24 antigen) for early
detection
Urine
Chlamydia and Gonorrhoea
Any other specific testing in addition to these that the
recipient may request will be chargeable. We do not test for
I.Q. Genetic testing for heritable diseases can be performed
with the donors consent and will be chargeable to the recipient.
All our Donors undergo rigorous fertility testing as well
as the full blood and urine screening once selected and retested
every 12 weeks after that. Any donors failing any part of
this testing process are withdrawn and your second chosen
donor will embark on the same process. All donors have to
produce valid photographic I.D. and 3 forms of proof of their
identification.
Donor candidates are disqualified and will not be
used if it is suspected or they are diagnosed with any S.T.D.
of any kind.

| Q: Who pays for the testing? |
| A: The testing
is initially part of the package price payable by you and
the retesting is paid for the duration of the program by MNI
only if the program runs its course for the monthly donations
from the start date with out any breaks. In the event of long
intervals between donations the company reserve the right
to re charge the recipient if necessary unless in exceptional
circumstances. Should a donor pull out of the program whilst
you are on a program MNI will either supply a tested donor
for your use or pay for a new selected donor. When donors
give notice to go away on holiday MNI can supply in most instances
a matched tested donor for that duration. If not you will
be given the option to take the same break with out losing
testing time and MNI will retest if necessary. |

| Q: Who covers the cost of the courier for deliveries? |
| A: All the delivery
costs are part of the particular donation package. Similarly,
all packages apart from the Gold pack offer singular donations.
Courier’s costs are built into this price of each singular
donation.
All aborted deliveries cancelled by you on the day of delivery
and less than 12 hours notice these will be charged as if
the donations went ahead.
From time to time when using a third party and traffic problems
arise, deliveries may be late any deliveries that are not
delivered or are delivered late through no fault of you own
will still be credited to you.

| Q: How can I/We be sure that our identity will not be given
to our matched Donor? |
| A: Next
to Heath and Safety a donor’s anonymity is the essence
and is paramount to our service. We endeavour to in all circumstances
protect both donors and recipients alike. In April 2005 the
change in legislation for all new FROZEN
sperm donors will be registered as known donors and records
will be kept.
Important: - Our service does not come under the
governing body Human Fertility and Embryology Authority (HFEA).
Because of this all donors registered with Fertility4Life.com
do not come under the new law as they donate FRESH sperm and
anonymity will still be protected.

| Q: Is there an upper age limit on recipients? |
| A: Yes-
We apply no restrictions but common sense and research
demonstrates how women’s chances of conception decrease
with age. Recipients over 45 years of age are not excluded
only their fertility testing is more intense and they are
advised conception could take much longer. We do recommend
they take the `Diamond Extra` 12 donation packs with fertility
consultations included. |

| Q: I am residing outside the United Kingdom – can I use
your service? |
| A: Yes -
through our extended thawing and frozen sperm service we can in fact provide our service globally and to date we have delivered to most countries in the northern and southern hemisphere.

| Q: Can I use a known donor of my own - can I select a known
Donor? |
| A: Yes- whilst
we operate a strict anonymous policy, you can use your own
donor if it is your preference as part of our program. We
can assist in all testing, screening, administration and co-ordinations
of the process. All donor profiles will show a` known donor`
question this means that they have the choice of willing to
be sought out for the well being of any child after that child
is 18 years old.
In both instance this will require a separate agreement. For

| Q: How many women have been successful so far? |
| A: Since our
launch we have had a success rate of 33% based on reported
conceptions, See home page for reported numbers. |

| Q: Can a heterosexual couple use you service? |
| A: Absolutely!
We do not discriminate against any women. |
|
| Q: How many times should I take donations per month? |
| A: Based on
fertility Doctors advice and now the numbers of reported conceptions
we recommend you use two inseminations per cycle to heighten
your chances of conception. |

| Q: What is the age limit for a woman wanting to conceive? |
| A: Conception
is far easier the younger you are but we do not have restrictions
on any fertile women that have undertaken testing
that our method will benefit them.
The older women become the longer it can take to conceive,
and their chances of suffering miscarriages is higher, but
this can unfortunately happen at any age too. Ageing of the
eggs can still happen even if you continue to ovulate and
still have periods increasing the risks of genetic disorders.

How do I register? |
| A: Click on the Donor Registration
button to register on Line. We do have manual registration
for those wish (contact us either by telephone on +44 (0) 20 8440 5760 or in writing to the address on our website). We ask you fill
in all fields if you do not know the answer to some questions
such as blood group please enter unknown. Failure
to complete all the fields will result in rejection of your
application.
Once you have registered and been approved you will hear back
within 48 hrs either by e-mail or in writing confirming you
registration with you unique I.D. Donor number. You are advise
to keep this safe as it is the number we record to protect

| A: This is because
there is no need. When you have registered once your I.D.
number is automatically assigned to you this is linked to
your profile details. You can login yourself to your Donor
profile by using your unique number which starts with a D.
You may have been advised to amend or add details. The company
cannot change any part of your profile for your added protection.
However we can disable at any time. And unless you are approved
your profile will not appear on our database viewed by recipients.

| Q:
How much do I get paid for each donation? |
| A: It is illegal to pay
for donated sperm in the U.K., however, Once selected, tested
and screened the recipient that selected you will provide
expenses from the introduction fee. Unreasonable request to
travel long distance MNI will cover these expenses for the
initial fertility tests on producing receipts.
(N.B. This initial testing of sperm will not incur
expenses paid)

What's involved in donating? |
| A: The procedure
is as follows:-
1. Complete our registration questionnaire/declaration either
on line or manually.
2. Your application will be either declined or approved within
48 hrs.
3. All applications will be approved with all the fields completed,
photographs attached to the application saves time in the
next process.
4. A summary of your profile will then be posted on the web
site for recipient selection. These details exclude name,
address, and photographs however due to the nature of the
method using fresh sperm your location will be revealed because
you need to be withing1 ½ hours travelling distance
to recipients.
5. Once selected by a recipient you will be contacted by us
to check your availability to be her special donor for the
duration of the program she has selected. You will be informed
of the approximate length of the program and asked a few questions
on your thoughts, i.e. if your donations were successful would
you be available to donate for any siblings.
6. The next stage would be a fertility test. This is performed
via a sperm donation.
It will consist of: - sperm Count and motility, abnormal forms
Ph level and MAR or antibody tests. The results from this
determine the next stage of screening. This is a simple blood
and urine test. With your ` welcome pack` you will receive
the relevant kits to provide us with the samples required
this includes prepaid bags to return to the Laboratory. The
screening consists of a range of S.T.D.`S.
N.B. You are advised to download a Donor Pack prior
to attending a testing centre.

Why is there a maximum age limit of 45 for donors? |
| A: In line with
all fertility organisations we have set ours the same. |

Is Donor testing compulsory? |
| A: Absolutely YES
this is paramount with no exceptions. |

| A: You will
have an initial sperm fertility Test carried out just the
once, testing and screening of blood and urine is carried

You and the recipient will have entered into an introduction
agreement between your selves with us as the go between. Our
service is offered with totally anonymity and every recipient

rights should your donations be successful at any time.

all circumstances protect both donors and recipients alike. Your right to anonymity has not been affected by the change in the law from April 2005 for all Frozen sperm donors in the U.K. Fertility4Life Fresh sperm Donors are not governed by this legislation or the H.F.E.A. who enforce this new law.

What personal details are available for the recipients to
view? |
| A: Your registration application
is for our office use only and are strictly private and confidential
this protected by the Data Protection Act 1984. Not at any
time will your name, address, e-mail or telephone numbers
be revealed to any recipient. All employees at Fertility4Life.com
Ltd sign a confidentiality agreement on their commencement
with the company. The recipients select from Hair and Eye
colouring, ethic Orgin, Country of Birth, Education, Hobbies
and Interest, Religion (if specified), Blood Group (If specified)
and you genetic health history and that of your family. Your
reason for donating which many cases is the reason this adheres
them to you as their selected donor.
Any photographs are held on record for the company's
use only. In the event we carry out on behalf of the recipient
to select a donor with facial characteristics. Hence a photograph
of you as a child is use for us. Except with our Crystal Package. |

Can a `Child` born out of donated sperm have access to the
biological Father? |
| A: NO- This is not possible
due to above protections in place unless agreed by both parties
prior to donations been used. This is preliminarily for the
well being of the child seeking his or hers genetic roots. |
